Metalytics Health provides a Member Portal through which you may complete Advance Directives, watch educational videos and more. As a member, you may be permitted to create an Authorized User Account in our Member Portal. You agree to accept all responsibility for all activities that occur under your account and password. You may not sell, transfer, or assign your account or allow others to use it. You are responsible for maintaining the confidentiality of any user name and/or a password that may be assigned to you or selected by you so others may not access any password-protected portion of our site using your account. We reserve the right, at our sole discretion and without notice to you, to terminate your account or restrict your access to it. Any Educational Content made available in the Member Portal is the copyrighted work owned or licensed by Metalytics Health. Use of the Educational Content is governed by the Terms and Conditions of the end user license agreement (License Agreement).
